Low Income Home Energy Assistance Program (LIHEAP)
Require the following information to receive assistance.
To apply for energy assistance, you must have:

· Valid Florida IDs for every household member 19 years of age or older.
· Social Security cards for everyone in the household.
· Current electric bill (April-September: assistance provided only for how
you cool your home).
· Custody papers for all minors living in the home who are not your
biological children.
· Income documentation for all household income for last 30 days:
Wages paid bi-weekly: bring last two check stubs
Wages paid weekly: bring last four check stubs
Wages paid once per month: bring last check stub

**Note: On rare occasions 5 check stubs may be required if paid weekly and
3 pay stubs may be required if paid bi-weekly and received within the
past 30 days.

· Social Security Award Letters must be dated for 2011, showing the
monthly income. If you do not have the current letter, you may have
one faxed to you by calling 1-800-772-1213.
· Unemployment Compensation: bring Wage Determination Transcript
showing weekly benefit amount.
· Child Support, Cash Assistance (TANF): Bring printout of income for
past 30 days.
· (NEW) Verification of Food Stamps (Food Stamp Award Letter)
· (NEW) Low-Income Housing (HUD/Section 8) Lease
· (NEW) Notarized letter from family member assisting you, if you report
NO INCOME.

**Please note that you must wait six months from the date you previously
received energy assistance to be eligible for LIHEAP assistance.
